Learn how to make plans for your future health care at any stage of life. “Concept of values” worksheets will be discussed as a guide to help people consider what is most important for their End of Life decisions and how to use them to be able to talk with family, friends, and medical providers about your wishes. End of Life options will be described, including discontinuing or declining medical treatments, palliative care, hospice care, voluntarily stopping eating and drinking (VSED) as well as California’s End of Life Option Act (2016), also called Medical Aid in Dying (MAiD). 

Presented by End of Life Choices California. End of Life Choices California (www.endoflifechoicesca.org) is a non-profit organization that provides free information, support, resources and guidance on end of life options to anyone and /or their family seeking help navigating a serious or terminal illness or simply wanting more information on future planning.
