Event Details

As a Community Health Navigator, my mission is to ensure that Medi-Cal members in our community have access to the resources and support they need to live healthy, fulfilling lives. This includes:

  • Assisting with obtaining food and housing benefits
  • Helping to schedule and follow up on doctor's appointments
  • Connecting members with primary care physicians and other healthcare providers
  • Linking individuals to community programs and social services

  • Developing personalized healthcare plans to address each member's unique needs
